What is entropion and ectropion?

Entropion and ectropion are two types of eyelid malpositions that affect the health of the eyes. Entropion is the turning in of the eyelid, which cause the eyelashes to rub against the cornea and conjunctiva. It causes irritation, redness and pain in eyes and also impairs vision. Ectropion is the condition in which the eyelid (usually lower) is turned outward and displaced away from the eye. In severe ectropion, the entire length of the eyelid is turned out. This causes exposure of the white area of the eye (sclera), leading to dryness and inflammation. In both the eyelid malpositions, drainage of tears is also affected, leading to watering from the eyes. Eyelid surgery is usually the solution to these malpositions in which eyelids are tightened by removing extra skin.

How is ectropion eyelid surgery performed?

Before surgery, the patient is given local anesthetic with or without intravenous sedation. Surgery depends on the cause for the eyelid malposition. If ectropion is due to aging and there are lax eyelid tissues (also called involutional ectropion), then eyelids are shortened and tightened by an incision on the outside corner of the eyelid, and reattaching it to the underlying tissue and upper eyelid (A surgery called the lateral tarsal strip).

If there is a scar due to previous injury (also called cicatricial ectropion), then a skin graft is used, taken from behind the ears or from upper eyelid for treatment. Before surgery, the patient is given local anesthetic. The operation usually takes from 30 minutes to an hour.

Recovery after surgery

After surgery, patients get swelling, redness and bruising around the eye, but recovery is very easy. The patient needs to wear an eye patch for 24 hours. Antibiotic ointments and cold compresses are given to patient periodically for 12 hours to reduce swelling and bruising. Stitches are removed in about a week and patients recover completely in about 2 weeks.

What are the symptoms of ectropion?

Normal blinking spreads the tears evenly in the eye, but when you have ectropion the tears are not drained properly, causing many symptoms like irritation in eyes due to stagnant tears, burning and chronic conjunctivitis, causing red & watery eyes. Ectropion also causes your eyes to feel dry and dirty. Sometimes there may be a discharge of mucous from the eyes and crusting of lids.

What are the complications of ectropion?

Ectropion leads to irritation and damage to the cornea. Dryness of eye may lead to corneal abrasions (scratches on the cornea or surface of the eye) and ulcers and may cause permanent loss of vision. Lubricating eyedrops are helpful in protecting the cornea, until proper treatment is performed.

What are the causes of ectropion?

Mainly older people are affected by ectropion. With age, the muscles around the eye become weak, but there can also be some other underlying cause. Estrogen can also be a result of scarring of the eyelid, like burning, infection, tumor or some injury. Other causes can be facial paralysis, skin problems, eyelid growth, drugs, previous surgery or it can be congenital.

How long will it take me to recover from ectropion eyelid surgery?

You can return to your routine activities four days after the surgery, but you should avoid wearing makeup and protect your face from sun for a few weeks. Stitches are removed in a week, and recovery takes about 2 weeks after surgery.

Is the surgery effective?

Most patients find this surgery very useful and there is very little if any discomfort after the surgery. Once you have gone through the surgery, you will be comfortable and the risk of corneal damage and vision loss will be greatly reduced. The results of surgery last longer, however, with you age, the eyelids will continue to get weaken again and the symptoms may appear again.
