Basal cell carcinoma is an epithelial cancer that occurs in cells in the deepest layer of the epidermis of the skin - the basal cells. The cancer can grow in the areas of the skin that are excessively exposed to the sunlight. This cancer does not usually spread to other parts of the body (i.e. is not metastasized) to become life threatening, yet it can bring about significant destruction of the surrounding tissues. For the diagnosis of this cancer, various techniques of biopsy (incisional as well as excisional biopsy) can be used and histological studies can be performed.

Surgical methods for the treatment of basal cell carcinoma (i.e. basal skin cancer surgery) include either standard surgical excision or Mohs surgery.

Standard excision removes the area containing cancerous growth in the surrounding normal growth region using a scalpel. The procedure is performed, after application of an anesthetic. The skin is stitched for recovery and the excised section is examined microscopically to ensure the removal of all malignant cells. This technique is less effective in comparison to Mohs technique and chances of cancer recurrence are more.

Mohs surgery entails removal of cancer with a thin layer of surrounding tissue and thorough examination of the tissue under microscope. The process of tissue removal and examination is repeated, until no more cancer cells are detected in the depth or the peripheries of the area. This technique has the highest cure rate and ensures minimal removal of the normal skin area, thereby reducing post-operative side effects and morbidity and assisting repair. Afterwards the wound is treatedthrough plastic surgery (primary wound closure through primary intent) or allowed to heal on its own (heal via granulation tissue or secondary intent).

After basal skin cancer surgery, some days are needed for recovery of skin and removal of stitches. Recovery time depends on the area of cancerous growth removed in the surgery. The recovery process is not that painful and requires no medication for pain. Some drugs may be taken in case the pain builds up.There are, however, some activity precautions like avoiding strenuous exercise or heavy lifting.
