- What is Rhinophyma?

Rhinophyma is a red, large, bulbous, ruddy nose, studded with nodules. It is caused by granulomatous infiltration, commonly due to untreated acne rosacea afflicting the nose over many years. Rhinophyma is characterised by prominent pores and a fibrous thickening of the nose, sometimes with papules. It is associated with the common skin condition acne rosacea. It can carry a strong psychological impact due to its effect on one’s personal appearance.


- What are the causes?

The condition is much more common in men than in women. The cause of rhinophyma is unknown, though it is generally regarded as a severe form of rosacea. Generally men who are in the age group of 30-50years are most likely to get affected. Individuals with light hair, skin, and eyes are at a greater risk for developing rhinophyma.

A significant family history of acne rosacea may also be an important risk factor that can contribute to development of rhinophyma.

It is a relatively rare disorder involving thickening of the skin on the nose and the presence of many oil glands.


- What are the symptoms?

Rhinophyma generally takes place in a later stage of acne rosacea. The patient may experience some of the following symptoms:

  • Very sensitive skin
  • Sudden flushing of face
  • Blotchy, red area in the center of the face
  • Telangiectasia: small blood vessels over the nose and cheeks get swollen up
  • Conjunctivitis: redness within the eyelids
  • Ocular rosacea: grittiness or burning in the eyes
  • Blepharitis: eyelids get inflamed

With further development in rosacea, the symptoms become even more intense, with the onset of rhinophyma. For instance, the oil glands and connective tissue on the nose may increase. The affected individual may experience the following changes in their nose:

  • Enlarged pores
  • Nose slowly grows into bulbous, swollen shape
  • Thickening of the outer layers of skin
  • Numerous oil glands
  • Rough, waxy, yellowish appearance
  • Reddish skin tone


- How is the diagnosis made?

Earlier stages of rosacea may be mistaken for acne or other skin conditions. Nevertheless, rhinophyma generally occurs after rosacea is identified.

Rhinophyma has unique features. Rarely your doctor may require doing a skin biopsy for confirming the diagnosis.


- What is the treatment?  

Rhinophyma may respond to medications if it is diagnosed in early stage. Erythromycin, tetracycline may assist in reducing redness and inflammation. Certain topical medications viz. tretinoin, azelaic acid, benzoyl peroxide may minimize swelling.

Surgical reshaping of the nose (Rhinoplasty) is the best known treatment for rhinophyma. This may be accomplished by laser, radiofrequency, or dermabrasion. Radiofrequency ablation gives the best results.
